Because of their mechanism of action, anticholinergic medications can interfere with the activity of A4icept and Exelon. This list has been reviewed by DQAC and SFC and will be used for assessing Arifept and Exelon requests. Congress renews the Older Americans Act with provisions extending the National Family Caregiver Support Program to people under age 65 who are living with Alzheimer's. Long-awaited results of a large, federally funded study on effectiveness of "atypical" antipsychotic drugs for behavioral dementia symptoms reinforce caution on use of these drugs for this purpose. All three "atypical" antipsychotic drugs in the study gave physicians on overall impression they were helping about 30 percent of the time. However, that benefit did not differ significantly from a placebo, which seemed to help about 20 percent of the time. Significant side effects were seen more frequently in those taking study drugs than in those receiving a placebo. The study appears in the New England Journal of Medicine. The FDA approves donepezil Aricept ; to treat symptoms of severe Alzheimer's. Donepezil was previously approved to treat mild to moderate Alzheimer's, and the new FDA action makes it the only drug currently approved to treat all Alzheimer stages. Researchers at the State University of New York at Stony Brook found in a small clinical trial that participants who took the Alzheimer's drug donepezil Aricept ; improved in several measures of cognitive function, including verbal memory. Thirty-Week Study In a study of 30 weeks duration, 473 patients were randomised to receive single daily doses of placebo, 5 mg day or 10 mg day of ARICEPT. The 30-week study was divided into a 24-week double-blind active treatment phase followed by a 6-week single-blind placebo washout period. The study was designed to compare 5 mg day or 10 mg day fixed doses of ARICEPT to placebo. However, to reduce the likelihood of cholinergic effects, the 10 mg day treatment was started following an initial 7-day treatment with 5 mg day doses. Effects on the ADAS-cog: Figure 1 illustrates the time course for the change from baseline in ADAS-cog scores for all three dose groups over the 30 weeks of the study. After 24 weeks of treatment; the mean differences in the ADAS-cog change scores for ARICEPT treated patients compared to the patients on placebo were 2.8 and 3.1 units for the 5 mg day and 10 mg day treatments, respectively. These differences were statistically significant. While the treatment effect size may appear to be slightly greater for the 10 mg day treatment, there was no statistically significant difference between the two active treatments. Following 6 weeks of placebo washout, scores on the ADAS-cog for both the ARICEPT treatment groups were indistinguishable from those patients who had received only placebo for 30 weeks. This suggests that the beneficial effects of ARICEPT abate over 6 weeks following discontinuation of treatment and do not represent a change in the underlying disease. There is no evidence of a rebound effect 6 weeks after abrupt discontinuation of therapy.

August 2002 "Pharmacotherapy for Alzheimer's Disease" Volume 24 Number 8 DIAGNOSIS Patients suffering from AD can experience a wide variety of cognitive deficits including senile dementia, memory loss, inability to express or comprehend written or spoken words, failure to recognize familiar objects, as well as problems with judgment and abstract thinking 1, 2. The criterion for a diagnosis of dementia requires the patient to exhibit memory decline plus at least one of the following features: aphasia, agnosia, or apraxia 4. Aphasia is impairment in verbal and written language. This is common in patients with vascular dementia, since the part of the brain that controls these abilities is largely affected. Agnosia is failure to recognize visual, auditory, olfactory, or tactile objects or impressions. For example, patients with auditory agnosia may not recognize the sound of a telephone. Apraxia is the inability to perform motor tasks that require many steps in the absence of motor or sensory impairment. These individuals are also unable to think abstractively, i.e. subtract 7's from a series of numbers 4. Since many of the same symptoms of dementia and AD could coincide with other medical conditions such as depression, or be the result of medication-related side effects or metabolic or nutritional deficiencies, the National Institute of Neurological and Communicative Disorders and Stroke and the Alzheimer's Disease and Related Disorders Association NINCDS-ADRDA ; formulated a diagnostic criteria for physicians evaluating patients with suspected AD.4 Once a complete physical and neurological medical examination have ruled out reversible causes of dementia, and AD is diagnosed, the level of disease severity is categorized as "probable or possible" AD. Probable AD is defined as a "clinical picture of dementia with absence of neurological abnormalities for which other causes cannot be found, " while possible AD is the "clinical picture of dementia with atypical features in the course of illness with secondary potential contributors not believed to be the primary cause of disease"2, 4. "Levels of acetylcholine, noradrenaline, serotonin, gammaaminobutyric acid GABA ; , glutamate, somatostatin, neuropeptide Y, and substance P have also been documented to be reduced in the brains of AD patients" 4. However, reductions in acetylcholine levels appear to be the most important 4. Originally it was thought that cholinergic agonists or cholinergic precursors such as choline or phosphatidylcholine would be the best agents to stimulate acetylcholine synthesis 4. Yet studies failed to report any positive cognitive benefits for AD patients 4. Whereas research using cholinesterase inhibitors provided more reliable improvements in memory and cognitive functioning 4. Hence drugs that supply choline by preventing its breakdown, like the acetylcholinesterase inhibitors tacrine Cognex ; , donepezil Aricept ; , galantamine Reminyl ; , and rivastigmine Exelon ; are mainstay treatments for this disease5. However, many of these agents have adverse side effects such as headache, dizziness, anorexia, nausea, and diarrhea, plus only provide modest symptomatic improvements for patients. In fact, approximately only one-third of patients who reach therapeutic levels of medication will show some benefit 7. In addition, acetylcholinesterase inhibitors increase cholinergic activity, which can impact many different organ systems in the body. Cholinergic agents can exert vagotonic effects on the sinoatrial and atrioventricular nodes causing bradycardia and syncope, bronchospasm, increased gastric acid secretions, increased sweating and salivation, bladder outflow obstruction, and exaggerated succinylcholine-type muscle relaxation during anesthesia 7. There also is an association between cholinergic agents and seizure activity. It is unclear whether the drug or the disease process is primarily responsible for this neurological manifestation. These agents should be used cautiously in patients with a history of cardiovascular disease, sick sinus syndrome, conduction abnormalities, pulmonary disease, seizures, and peptic or duodenal ulcer disease. Patients and caregivers should be counseled about the potential adverse effects associated with these medications as well as suggestions to reduce their severity.
