Using rat Kv1.4 as a model KCh, together with chimera and mutants carrying deletions within the carboxy terminus, we have identified a critical region residues 608-617 ; controlling Kv1.4 internalisation. To examine the autonomy of this region, the internalisation and expression levels of a novel reporter protein, PIN-G, expressing varying lengths of Kv1.4 carboxy terminus, was also investigated and these parameters compared and contrasted with corresponding mutations in Kv1.4. Surprisingly, the region we have identified lacks a canonical internalisation signal but contains a sequence homologous to a novel motif implicated in ubiquitination of the growth hormone receptor. From patients registered in the JET study whose intracranial arterial occlusive disease was treated surgically or medically. PATIENTS AND METHODS Patients We recruited 13 male and 3 female patients aged 60.2 6.6 years mean s.d. ; with intracranial artery occlusive disease, from the JET study. Cranial angiography confirmed occlusion of bilateral middle cerebral artery in 1 patient, of unilateral internal cervical artery in 5, and of unilateral middle cerebral artery in 10. Eight of the patients were selected at random for surgery for intracranial artery occlusion and the others underwent a medical approach. We conducted SPECT studies of the 16 patients before and 1, 6, 12, months after therapy, and obtained 16, 13, 15, and 10 datasets respectively. Methods We performed a paired CBF SPECT study with resting and vasodilative drug acetazolamide: Diamx ; challenge according to the JET study protocol of the patients before and after therapy. The CBF image set was quantified using autoradiography ARG ; method.8, 9 All patients were intravenously administered 222 MBq of 123I-IMP ; for one minute at a constant rate. We acquired SPECT data 20 min after the injection. Blood was sampled from the antecubital artery 10 min after the injection. For SPECT with drug challenge, 15 mg kg of Diamoox was injected 7 min before 123I-IMP. The resolution of the SPECT system was 10.4 mm of full-wide half maximum, and the system consisted of a three-headed gamma camera GCA-9300A HG Toshiba Co., Tokyo ; equipped with low-energy highresolution fanbeam collimators, and a processing computer GMS-5500A DI Toshiba Co., Tokyo ; . Projection data were obtained from each camera in 10 rotations 128 matrix for 4 angles of 120 at 4 sec per angle of rotation ; and with triple photopeaks for triple energy window scatter correction 160 keV, 24% and 134, 172 keV, 3% ; .10 Image reconstruction Tomographic images were reconstructed from the 128 matrix in the transverse plane using filtered backprojection algorithms and a Butterworth filter cutoff of 0.10 cycles pixel and order of 8 ; . Contraindications -- Situations in which sodium and or potassium serum levels are depressed, in kidney and liver disease or dysfunction, suprarenal gland failure and hyperchloremic acidosis. Long-term administration is contraindicated in chronic noncongestive angle closure glaucoma. Warning--Although teratogenic and embryocidal effects demonstrated in mice at more than ten times the equivalent therapeutic doses have not been evidenced in humans, DIAMOX Acetazolamide should not be used in pregnancy, especially during the first trimester, unless the expected benefits outweigh these potential adverse effects. Precautions--Increasing the dose may increase drowsiness and paresthesia and decrease diuresis. Reactions common to sulfonamides may occur: fever, rash, crystalluria, renal calculus, bone marrow depression, thrombocytopenic purpura, hemolytic anemia, leukopenia, pancytopenia, agranulocytosis. If such occur, discontinue drug and institute appropriate therapy. Side Effects--During short-term therapy: paresthesias, loss of appetite, polyuria, drowsiness, confusion. In long-term therapy an acidotic state may supervene. Transient myopia has been reported. Other occasional reactions: urticaria, melena, hematuria, glycosuria, hepatic insufficiency, flaccid paralysis, convulsions.

Abstract. Actual tuberculosis control strategies focus on the treatment of active, infectious cases. In this work we explore the effect of a complementary strategy consisting in the administration of preventive therapy to recently infected but not infectious ; contacts of identified active cases. We show that even when a small fraction of the infected contacts are effectively treated, significant reductions in the incidence of active tuberculosis are achieved. More important such a combined strategy may be less costly than the currentlyemployed strategy and evista.

The streets are made of sand and are therefore totally quiet to walk along, and in the afternoon, when most sane people are inside cowering from the sun, Timbuktu is reverently serene!

Trew Hammersmith Hospital, Reproductive Medicine, London, United Kingdom Introduction: This study develops a new concept in understanding the importance of the interaction between maternal pro-inflammatory cells and invading trophoblast cells in the early pregnancy decidua. Although likely to be a key factor influencing the success of implantation, it is still not known if pro-inflammatory cells are beneficial or harmful to the pregnancy. Studies are limited because histological specimens obviously cannot provide information about ongoing pregnancies, and the process begins at 3-4 weeks gestation, usually long before spontaneous pregnancies are recognised. We have developed a novel approach by using serum C-reactive protein CRP ; as a potential marker of the early pregnancy inflammatory response. Women undergoing treatment by in vitro fertilisation IVF ; provided the basis of a well-controlled and closely followed-up study. Methods: Successive women undergoing treatment by IVF were recruited. Blood was taken at 4 weeks gestation 2 weeks after egg collection ; for serum beta-human chorionic gonadotrophin hCG ; and for CRP. Women with positive pregnancy tests were followed up by serial transvaginal ultrasound scans until 8 weeks gestation. While it was possible that factors involved in infertility and the treatment cycles themselves could affect CRP levels, women who failed to get pregnant provided closely matched controls. Results: Women with normal on-going pregnancies n 40 ; had significantly higher CRP levels 3.68mg l ; than those who were not pregnant n 95; 1.495mg l; p 0.0001 ; . The difference persisted after accounting for possible confounding factors including pre-treatment CRP levels, age, body mass index, cause of infertility, smoking, agonist or antagonist stimulation cycles, dose of FSH received, peak levels of oestradiol, number of eggs collected, multiple pregnancy, and development of ovarian hyperstimulation syndrome OHSS ; . Pregnant women with OHSS n 6 ; had significantly higher CRP levels than pregnant women without OHSS p 0.01 ; . Women with positive pregnancy tests hCG 2iu l ; who did not have on-going pregnancies n 54 ; had CRP levels that were not significantly different to nonpregnant controls, and significantly lower than the ongoing pregnancy group p 0.001 ; . Conclusions: This study has demonstrated for the first time that a maternal systemic inflammatory response, known to characterise normal pregnancy, is established as early as 4 weeks gestation. CRP levels in pregnant women who developed OHSS were even higher, consistent with the inflammatory hypothesis for that condition. In contrast, early pregnancy loss was associated with low CRP levels. These findings raise the hypothesis that an inflammatory response is an essential feature of implantation and that, in some cases, an absent inflammatory response could be the underlying cause of early pregnancy loss. This hypothesis also provides a possible mechanism for the reputedly harmful effects of non-steroidal anti-inflammatory drugs in early pregnancy.

Be sure to carry a photocopy of your passport in a separate location, for security's sake. It is also a good idea to leave one with somebody at home. If you already have a passport, check to make sure it is still current they expire every 10 years ; . Passports can be renewed through the mail. If you are getting a passport for the first time, you must submit your application in person. Allow at least 6 weeks. Health: Peru is a very safe place to enjoy on vacation. There are few serious health concerns, and no immunizations are required. We eat in only the best restaurants and our own cooks are carefully sanitation trained. We have had good results with Diamo as an aid in acclimatization and alleviation of the effects of altitude. Language: You can often times find someone who speaks English in the more populated areas, but in the rural areas where we will be traveling, comprehension of English will be rare. Although Spanish is considered "essential" in Peru, Quechua qhesh-wa ; , the language of the Incan Empire, is the official one. Conditioning: Additional aerobic conditioning is unnecessary for anyone who already exercises regularly and is in sufficient condition to walk up and down hills at a moderate pace for an hour at a time while conversing. About two weeks before your trip it would be ideal if you could fit in several longer day hikes, 2 to 4 hours, with your pack packed with all your clothes and equipment for the trip. If you don't already exercise, you should start a moderate exercise program at least a couple of months before your trip. For example, you might start walking 3 or 4 times a week for half an hour, at a pace that is brisk enough to make you breathe deeply. Hills are good for this. Other options are working out at a health club on a stairmaster or treadmill, aerobics classes, cross country skiing or any activity that makes you breathe deeply. Gradually increase the level of your activity until you can hike up and down hills for an hour at a time. Again, fitting in some 2 to 4 hour day hikes with your pack loaded would be ideal. Accommodations and meals: This is a safari style quality adventure using pack-stock to carry all gear and camping amenities. Comfortable camps offer delicious meals prepared from fresh meats, grains and vegetables served in our large dining tent with tables and chairs. One or two persons are assigned a weather-tight, four person sleeping tent. Each day begins with a pan of hot water and coffee or tea served at your tent. Before the evening meal, we enjoy happy hour with popcorn and assorted beverages. Electrical Appliances: Peru runs on 220V, 60HzHz. Be sure to bring a plug adapter for any electrical items you travel with - plugs have two round pins. Some specialty travel stores carry adapters and can help you chose the right one. Spending Money: The Peruvian currency is the Nuevo Sol 100 centimos ; , and is currently equal to 0.29 U.S. dollars. Travelers checks are not as easily converted as cash, nor as convenient as credit cards, but they are a safe way of carrying money. ATMs are convenient, fairly available, and often offer the best exchange rate check with your bank to make sure that yours will work there ; . Visa is widely accepted; and other cards are pretty much so. Once we leave Cuzco you will need cash. Credit cards are not as frequently accepted in rural areas, only in towns. You will need money for snacks, beverages, personal excursions, gifts, and other items of a personal nature. Weather: May through September is considered the best time to visit the mountains lucky us! ; . The temperatures can vary significantly, depending on what elevation we're at. It can get into the 30s at night and be in the 70s during the day. When we descend into the cloud forest temperatures can rise into the 80s. This is normally the dry season, but it can rain or snow ; . Please come prepared. Packing: Traveling light enhances the feeling of freedom on vacation. However, do not forget your raingear. You will need a daypack to carry water, raingear and snacks when hiking. There can be pickpockets in crowded markets, and you may want to carry most of your money in a moneybelt or neck wallet when in those settings. Although Peru is generally safe, we recommend leaving valuables at home.
