Adjuvant Analgesics - Antidepressants Until 1999, tricyclic antidepressants TCAs ; such as amitriptyline Elavil ; were considered by many to be the "gold standard" for the treatment of neuropathic pain. However, following the publication of the well-controlled studies of gabapentin and the lidocaine patch, the TCAs have lost their priority in the neuropathic pain treatment algorithm. Selective serotonin reuptake inhibitors SSRIs ; such as fluoxetine Prozac ; and paroxetine Paxil ; have fared inconsistently in clinical trials. For other antidepressants, including the selective norepinephrine and serotonin reuptake inhibitor SNSRI ; class, the data currently available are too limited to allow the effectiveness of these drugs to be evaluated. The mechanism of action of the TCAs with respect to neuropathic pain has traditionally been considered a result of their serotonergic and noradrenergic activity within the descending pain-modulating system of the CNS. Recent data from animal studies suggest that the analgesic effects of these drugs in neuropathic pain may be, at least in part, a result of sodium channel-blocking activity at the site of ectopic discharge in peripheral nerves. Many controlled clinical studies have established that TCAs can produce significant pain relief in PHN and diabetic neuropathy, and that these analgesic effects are independent of the antidepressant effects. These drugs provide relief more quickly and at lower dosages when used for pain than for depression. Despite this apparent difference in effective dosage levels and onset of action, a TCA trial cannot be considered adequate unless the drug has been titrated until satisfactory pain relief or dose-limiting side effects occur and trazodone.
